What’s in the template
Every column, in the order it appears in the file, and how to complete it so the plan survives client review.
Item number and activity
One line per verifiable activity, sequenced in the order work is executed. If an activity cannot be inspected or witnessed, it does not belong on the ITP.
Reference document
Cite the drawing revision, specification section, code clause or purchase-order requirement the activity is verified against — never a generic standard number on its own.
Acceptance criteria
State the measurable pass condition (tolerance, DFT range, code acceptance class). An ITP line without acceptance criteria produces a signature, not evidence.
Inspection or test method
Visual, dimensional, RT/UT/MT/PT, hydro, functional or document review. The method must be capable of proving the acceptance criterion.
Frequency and extent
100%, first-article, per lot, or a sampling percentage tied to the specification. Sampling that is not defined here becomes sampling that is disputed later.
Responsibility
The named role that performs the activity — QC inspector, CWI, NDT Level II, coating inspector, document control.
Intervention columns (Contractor, Vendor, TPI, Client)
One column per party, coded H (hold), W (witness), S (surveillance) or R (records review). This is the column clients negotiate, so agree it before the first hold point falls due.
Verifying record
Name the record produced — fit-up report, VT report, NDT report, DFT report, pressure-test certificate. These names become the MRB index entries at turnover.
H, W, S and R explained
- H
- Hold point. Work may not proceed past this point until the party attends and releases it in writing. Non-negotiable and time-critical — give the agreed notice period.
- W
- Witness point. The party is notified and may attend. Work may proceed if they do not attend after due notice, provided the notification is recorded.
- S
- Surveillance. Periodic, often unannounced verification of an ongoing activity rather than a single event.
- R
- Review of records. No attendance required; the party reviews the resulting documentation.

What is an inspection and test plan template?
A controlled table that lists every inspection and test activity for a scope of work, with the reference document, acceptance criteria, method, frequency, responsible party, hold/witness/surveillance/review codes for each party, and the record that proves the result. The template gives you that structure pre-built so you only fill in project-specific content.
Is this ITP template free to use commercially?
Yes. Download it, add your own document number, revision and logo, and use it on your projects. It is a starting point rather than a controlled document — bring it under your own document control before issuing it to a client or vendor.
What do H, W, S and R mean on an ITP?
H is a hold point: work stops until the party attends and releases it. W is a witness point: the party may attend after notification and work may continue if they do not. S is surveillance: periodic monitoring of the activity. R is review of records only, with no attendance.
Who approves an inspection and test plan?
The contractor or vendor prepares it, the project QA/QC manager approves it internally, and the client (and any third-party inspection agency or authority having jurisdiction) reviews and signs the intervention columns before work starts.
