What is an ITP (Inspection and Test Plan)?

An ITP is the controlled document that lists every inspection and test for a scope of work, in sequence, with the acceptance criteria, the reference clause, the responsible party and the record produced at each step.

ITP meaning in one paragraph

An Inspection and Test Plan (ITP) is a controlled quality document that sets out, activity by activity and in the order the work happens, what will be inspected or tested, against which code or specification, to what acceptance criteria, by whom, what objective evidence is produced, and at which points work must stop for verification. It is sometimes called a quality control plan or QCP; on regulated projects the ITP is the document an auditor opens first, because it is where the promised verification and the delivered verification can be compared line by line.

Formats vary by owner and by sector, but a reviewable ITP carries these columns. Anything missing here is a gap someone will argue about later.

Notice what makes these lines usable: each one is a single physical operation, the clause is specific enough to look up, the acceptance criterion is measurable, and the record named in the last column is the exact document that will land in the data book. An inspector who has never seen the job can pick this up and verify it.

Hold, witness, review and monitor — the difference

Hold point (H)

Work cannot proceed past this activity until the nominated party attends and signs. Reserve holds for irreversible or safety-critical steps.

Witness point (W)

The party is notified in advance and may attend; if they do not attend within the notice window, work proceeds and is recorded.

Review point (R)

A documentation check after the fact — certificates, reports and calibration records reviewed against the acceptance criteria.

Monitor / surveillance (M)

Unannounced or periodic observation of an ongoing activity, typically welding or coating, without stopping the work.

Misassigning these is the most common ITP defect we find. Marking everything a hold point buries the schedule in waiting time; marking critical NDE as a review point lets a defect ship. The notice period for witness points — typically 24 to 72 hours — belongs in the ITP itself, not in an email thread.

Who owns it

Who writes, approves and holds the ITP

  • The party performing the work — fabricator, vendor or contractor — prepares the ITP, because they own the process and its sequence.
  • The owner or owner's engineer reviews it, adds their own hold and witness points, and issues it Code 1 / Approved before work starts.
  • A third-party inspection agency verifies the marked points independently and signs the ITP at attendance.
  • On EPC projects the EPC contractor consolidates vendor ITPs into a package-level master ITP that indexes, but does not replace, each vendor's plan.
